The most important factor to consider in the treatment of malocclusion is the function and dysfunction of the craniomandibular system (CMS). The role of CMS is to maintain the mandibular position in a physiological position. There is a risk that the mandibular position is easily displaced in a realigned human maxillofacial skull. It is the occlusion that most affects the mandibular position. Therefore, it is always necessary to consider the mandibular position in orthodontic treatment. Especially in the diagnosis and treatment planning of malocclusion, the guideline regarding the direction and specific position with respect to the mandibular position is important. Orthodontic treatment is an effective means to improve the mandibular position by controlling the vertical dimension and occlusal plane, improving the shape of the dental arch, and optimizing occlusal guidance. Orthodontic treatment has become the most effective treatment option for CMS dysfunction.

One of the most frustrating orthodontic cases is a full Class II malocclusion. Based on the observations of developmental processes of malocclusions, full Class II subjects showed that adaptation of the mandible is not enough forward or the mandible stays in a backward position. Therefore, we should recognize that fundamental problems in full Class II cases are the retrognathic mandible, because of not enough occlusal support due to steep posterior occlusal plane. The proper treatment approach should be to control the occlusal plane, vertical dimension, arch coordination upper and lower dentition, providing retrusive guidance, and inter-coronal opening space in front teeth. It should be careful to avoid overuse of Class II elastics, which causes the elimination of space for mandibular adaptation. Class III elastics are often required for retraction and/or upright of the mandibular anterior teeth to reposition the mandible in a forward position.

El desplazamiento lateral mandibular (MLD) se caracteriza comúnmente por tener una desviación del mentón de la línea media facial, un desplazamiento de la mandíbula hacia un lado, la presencia de una mordida cruzada en la región posterior, asimetría facial, así como una discrepancia esquelética de la línea media. El conocimiento y la comprensión de MLD es muy importante en el tratamiento ortodóntico de las maloclusiones, ya que un número considerable de maloclusiones implican algún grado de asimetría facial.
