Anterior open bite is a very challenging malocclusion even for excellent orthodontists. It is very difficult to diagnose differentially and to retain the post-treatment result well, because one case may have several etiologic factors. Since MRI and CT were used to screen the etiologic factors, I have realized once more that ‘open bite’ is one sign of different diseases caused by thumb sucking, tongue thrusting, low tongue posture, mouth breathing, macroglossia, temporomandibular disease and ankylosis of anterior teeth. It cannot be overemphasized to try to find and to remove the causing factors first. According to the causes the open bite should be treated differentially.

Seminar Kee-Joon Lee. MARPE: the beginning and current status of orthopedic expansion in all ages for nonextraction / nonsurgical treatment.
Transverse discrepancy is not easily recognized by the patient and the orthodontic envelope of discrepancy is reportedly narrow in the transverse direction, which is why surgically assisted palatal expansion is recommended for the correction of maxillary transverse deficiencies in grown-up patients. Due to the inherent invasiveness and morbidity, an alternative has always been sought for.
In this lecture, the biomechanical rationale, appliance fabrication and clinical manipulation of the miniscrew-assisted RPE (MARPE) will be extensively discussed, with special focus on the underlying suture biology and the biomechanics of orthopedic expansion, to explain the validity of the appliance design and expansion protocol. A number of clinical cases indicating the use of MARPE including Class III, asymmetry nonsurgical cases will be demonstrated. Troubleshooting in failure cases will also be explained. According to the theoretical background and clinical examples, it appears that deficiency in the maxillary posterior region, not necessarily the anterior region necessitates true orthopedic expansion which secures the quality and stability of occlusal outcomes.
